Its active ingredient is amiodarone.
Certain medicines- such as amiodarone, corticosteroids and high doses of vitamins B6 and B12.
The main representative of this class is amiodarone(Cordaron).
Amiodarone- shown in patients who, in addition to tachyarrhythmia, have structural changes in the heart.
Such a condition requires cardioversion and/ or antiarrhythmic therapy(most often amiodarone).
Beta-blockers and amiodarone are the most effective and tested means of prevention of AF related to CABG.
It is recommended to use a statin not metabolised by CYP 3A4 when given with amiodarone.
Medication- daily tablets of medication such as amiodarone can help prevent episodes by slowing down the electrical impulses in your heart.
Those with thyroid cancer or onmedications affecting thyroid function, such as amiodarone, were excluded.
Amiodarone has been evaluated in several studies on its effectiveness in treating patients with an additional way of impulses.
Medication that may be administered may include adrenaline(epinephrine), amiodarone, atropine, bicarbonate, calcium, potassium and magnesium.
Amiodarone and DEA have also a potential to inhibit some transporters such as P-glycoprotein and organic cation transporter(OCT2).
However, as a primary prevention of ventricularfibrillation in patients with left ventricular depressions, amiodarone is most often not used.
For patients who receive amiodarone or verapamil concurrently with Simvatin® the daily dose of Simvatin® must not exceed 20 mg.
For this purpose, an electrical ormedicinal cardioversion is used with such drugs as propafenone, amiodarone, nibentan, procaineamide.
Class III preparations(amiodarone, sotalol) are used in various forms of rhythm disturbance, but only in cases where other agents are inactive.
With the development of neuropathy orneuritis of the optic nerve caused by amiodarone, the drug must be canceled because of the risk of developing blindness.
Caution should be exercised when aliskiren is administered with ketoconazole or other moderate P-glycoprotein inhibitors(itraconazole, clarithromycin, telithromycin, erythromycin, or amiodarone).
Caution must be exercised if the concomitant use of fluconazole and amiodarone is necessary, notably with high dose fluconazole(800 mg).
Also there may be an increased risk of inducingventricular arrhythmias if hydroxychloroquine is used concomitantly with other arrhythmogenic drugs, such as amiodarone and moxifloxacin.
With simultaneous use with class I antiarrhythmic drugs, amiodarone, it is possible to enhance the negative inotropic effect and suppress AV conductivity.
Amiodarone, when used during pregnancy, is associated with serious side effects, such as fetal hypothyroidism(9% of newborn mothers receiving chronic amiodarone therapy), hyperthyroidism and goiter.
There is no reason in such cases to discontinue amiodarone treatment if there is no clinical or further biological(usTSH) evidence of thyroid disease.
In acute ventricular fibrillation and ventricular arrhythmia, drugs(eg, vasopressin, adrenaline, amiodarone) are used after three defibrillation attempts to restore normal rhythm.
Active studies have also shown that amiodarone does not consistently prolong the refractory period of DPP and, as a result, can not be considered effective in preventing sudden death in all patients with WPW syndrome.
Specific interaction studies with local anesthetics andanti-arrhythmic drugs class III(e.g. amiodarone) have not been performed, but caution is advised.
Patients treated with anti-arrhythmic drugs class III(e.g. amiodarone) should be kept under close surveillance and ECG monitoring considered, since cardiac effects may be additive.
Patients with atrial fibrillation on the background of congestive heartfailure are advisable to carry out cardioversion Amiodarone, since this drug not only reduces the heart rate, but also has a minimal inotropic effect.
Thus, in combination with the well-known toxicity to internal organs characteristic of amiodarone and the high rate of discontinuation of this drug due to noncardial side effects, amiodarone usually does not play an important role in the treatment of patients with DPP.
Separate study of the interaction of local anesthetics andantiarrhythmic drugs of the III class(for example amiodarone) has not been carried out, nevertheless, it is recommended to use caution when they are used simultaneously(see"Special instructions").
