Permanently discontinue trastuzumab emtansine in the absence of another likely cause for the elevation of liver enzymes and bilirubin, e.g. liver metastasis or concomitant medication.

The following risks, including appropriate action to be taken, are discussed in greater detail in section 4.4: cardiac failure events, hypertension, thyroid dysfunction, arterial thromboembolic events, venous thromboembolic events, elevation of haemoglobin or haematocrit, haemorrhage, gastrointestinal perforation and fistula formation, wound healing complications, PRES,proteinuria, and elevation of liver enzymes.
Adverse reactions, included reversible elevation of liver enzymes and haematological events; in addition, fever, infection, dehydration and thrombosis/embolism were also reported.
Elevation of liver enzymes, fatigue, fever, malaise, influenza-like symptoms, bronchospasm-like symptoms, serum sickness, thrombocytopenia- dizziness, headache, paresthesia- nausea, vomiting, diarrhea, abdominal pain- arthralgia, myalgia- rash, pruritus,- hypotension, syncope- paralysis(Bell's palsy), neuropathy, neuritis(including Guillain Barre Syndrome, myelitis including transverse myelitis), encephalitis.
This evaluation should differentiate between patients with no risk factors and patients with risk factors, such as excessive prior alcohol consumption,persistent elevation of liver enzymes, history of liver disease, family history of a hereditary liver disease, diabetes mellitus, obesity and history of significant exposure to hepatotoxic medicinal product or chemicals, as well as prolonged methotrexate treatment or a cumulative total dose of 1.5 g or more.
Other findings included an effect on the liver( elevation of liver transaminases) and retinal function, and potential for phospholipidosis in multiple organs without correlative toxicities.
If there is evidence of new orworsening liver dysfunction(including clinically significant elevation of liver enzymes and/or symptoms such as fatigue, anorexia, nausea, jaundice, dark urine, liver tenderness, hepatomegaly) in patients using Symtuza, interruption or discontinuation of treatment should be considered promptly(see section 5.3).
If there is evidence of new orworsening liver dysfunction(including clinically significant elevation of liver enzymes and/or symptoms such as fatigue, anorexia, nausea, jaundice, dark urine, liver tenderness, hepatomegaly) in patients using darunavir/ritonavir, interruption or discontinuation of treatment should be considered promptly.
If there is evidence of new orworsening liver dysfunction(including clinically significant elevation of liver enzymes and/or symptoms such as fatigue, anorexia, nausea, jaundice, dark urine, liver tenderness, hepatomegaly) in patients using darunavir used in combination with cobicistat or low dose ritonavir, interruption or discontinuation of treatment should be considered promptly.
